Medicare Participation
As a member of DaVita VillageHealth, you may go to any provider that accepts Medicare and agrees to see DaVita VillageHealth members.

DaVita VillageHealth has developed a network of doctors, specialists, and hospitals for members to choose from. However, as a Point of Service (POS) option, you are not limited to using providers within this network. You may continue to see your current physicians as long as your physicians accept Medicare and will accept payment from DaVita VillageHealth.

Whichever physician you choose, you will have the same benefits and out-of-pocket costs. There is no referral required to see a specialist or other provider who performs services covered by the DaVita VillageHealth Plan.

Remember — You may go to any provider that is Medicare-approved and will accept payment from DaVita VillageHealth.

The providers in the DaVita VillageHealth network can change periodically. If a change in the network occurs that directly affects you, DaVita VillageHealth will provide you with written notice of the change.

DaVita VillageHealth cannot ensure that you will be able to see a specific provider just because they are part of the network. Sometimes, providers experience a lack of capacity to take on new patients or are unavailable for a variety of reasons. If you have questions about the availability of any provider in our network, please contact our Customer Service Representatives.

NOTE: Members must use network pharmacies to access their prescription drug benefit except under non-routine circumstances when they cannot reasonably use network pharmacies. DaVita VillageHealth offers in-network national coverage, which means that our pharmacy network is nation-wide. You pay the same amount for your prescription drugs as long as you get them at an in-network pharmacy, even if it is located outside of your local service area (for instance when you travel). Disclaimer

If you are eligible for Medicaid, the state may be required to cover Medicare cost-sharing that you would otherwise be required to pay. These amounts may differ based on what kind of Medicaid coverage you have. Benefits, co-payments, coinsurance and premiums may change on January 1, 2009. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party.
