Finding a Provider

As a member of DaVita VillageHealth, you may go to any provider that accepts Medicare and agrees to see DaVita VillageHealth members.

DaVita VillageHealth has developed a network of doctors, specialists, and hospitals for members to choose from. However, as a Point of Service (POS) option, you are not limited to using providers within this network. You may continue to see your current Nephrologist and Primary Care Physician as long as your physicians accept Medicare and agree to see DaVita VillageHealth members.

Likewise, nothing will change at your dialysis facility. The nurses, social worker, and dietitian at your current dialysis facility will still be closely involved in your case.

Whichever providers you choose — in or out of network so long as they accept Medicare — you will have the same benefits and out-of-pocket costs. There is no referral required to see a specialist or other provider who performs services covered by DaVita VillageHealth.
