Company Overview

VillageHealth is comprised of a dedicated team of specially trained nurses and professionals providing integrated care management to patients with kidney disease throughout the US.  We provide services to help improve the lives of our patients by working with them to prevent complications, reduce the number of avoidable hospitalizations and improve overall health.

VillageHealth is not a one-size-fits-all approach, but our integrated care management services are formulated to meet the unique needs of each patient. We work with you, your PCP and the entire care team to develop a personalized plan of care.  Working together we can enhance the care of patients with kidney disease.


VillageHealth ICM Approach

Using an Integrated Care Management (ICM) approach, VillageHealth strives to make sure that valuable resources are used effectively, creating an enhanced patient experience and improved clinical outcomes. The VillageHealth ICM approach

  • Coordinates care with the patient, physician(s) and provider
  • Educates and engages patients to take an active role in their health care
  • Provides a patient centric approach to care backed by proven results
