Your Village Health Team
One of the biggest differences you will have with DaVita VillageHealth is a dedicated team of experts focused on making sure that, as a person with end stage renal disease, you get the care and attention you need.The process starts with an in-depth, personal interview with you and your caregivers when you first join the plan. Our team will empower you with the round-the-clock support; guidance and education you need to live well and stay strong.
The VillageHealth Team is lead by your Nephrologist. Other members include:
- Your VillageHealth Nurse:
- Serves as your healthcare advisor
- Gets to know you and develops a personalized care plan
- Monitors your health status
- Is available round-the-clock, seven days a week
- Helps you access the services you need, when you need them
- Clinical Pharmacist
- Regularly reviews your medications
- Ensures you understand the purpose of your medications
- Coordinates your medications, as needed, with your Nephrologist and other physicians
- Customer Service Representative
- Available to assist you with any questions regarding your benefits or other concerns
- Assist in completing any necessary forms
