Finding a Pharmacy

DaVita VillageHealth provides the prescription drug benefit through a national network of pharmacies. The Plan offers in-network national coverage, which means that our pharmacy network is nation-wide. You pay the same amount for your prescription drugs as long as you get them at an in-network pharmacy, even if it is located outside of your local service area (for instance when you travel).

Members must use network pharmacies to access their prescription drug benefit except under non-routine circumstances when they cannot reasonably use in-network pharmacies or no pharmacy is located in a reasonable proximity to your temporary location. Because DaVita VillageHealth offers in-network national coverage, in most instances, there will be an in-network pharmacy available to supply your medications, even if you are temporarily outside of your local service area (for instance when you travel).

In-Network Pharmacies

To find an in-network pharmacy or to see if your current pharmacy is part of the DaVita VillageHealth network, click here to view or download our Pharmacy Directory.

The directory provides you with the name, address, and phone number of those pharmacies that are contracted with DaVita VillageHealth, both in the service area and nationally. The type of pharmacy is also displayed; retail, mail order, or long term care.

If the pharmacy is part of a national chain, a toll-free number (including a TTY/TDD number) may be displayed in the directory instead of each location. This number may be called to obtain the pharmacy location nearest to you.

This directory is periodically updated. Pharmacies listed may not be open or at the same location as listed in the directory. Occasionally, the pharmacy may no longer be participating in the network. Be sure to confirm that the pharmacy is part of DaVita VillageHealth's network before obtaining services.

Out-of-Network Pharmacies

Benefits at out-of-network pharmacies are limited and covered only under special circumstances when a member cannot reasonably use in-network pharmacies.

When services are received from an out-of-network pharmacy, you may be asked to pay for the full cost of the prescription (rather than paying just your co-payment). You may ask us to reimburse you for our share of the cost by submitting a claim. You should submit a claim to us if you fill a prescription at an out-of-network pharmacy, as any amount you pay for covered prescriptions will help you qualify for catastrophic coverage.
