Begin your path to better living by enrolling in DaVita VillageHealth

If you are interested in learning more about DaVita VillageHealth or enrolling, you have several different options:
  • You may contact us and ask for a personal visit from a Sales Representative. During this visit, the Sales Representative will discuss with you the details of the DaVita VillageHealth plan and provide you with the information you need to make a choice in coverage that is best for you.

  • You may contact our Customer Service Department and request that a Pre-Enrollment Packet be mailed to your home. This packet contains a variety of information, providing you with facts you need to make an informed choice about your coverage as well as an enrollment application and an envelope.

  • You may complete a Lead Card, available at many dialysis facilities in the areas we offer DaVita VillageHealth, requesting that we mail you a Pre-Enrollment Packet or contact you by telephone.
  • You may download a copy of our Application for Enrollment with instructions, fill it out completely, and return to us at: DaVita VillageHealth

  • Please complete the separate Coordination of Benefits (COB) Survey if you have other health or pharmacy coverage and mail with your Enrollment Form application.
