DaVita VillageHealth is a Medicare Advantage Special Needs Plan with a Medicare Contract. You are eligible to enroll if:- You're entitled to Medicare Part A and enrolled in Medicare Part B
- You have end stage renal disease (ESRD)
- You reside in one of the service areas served by DaVita VillageHealth

Other network and benefit limitations, copayments and restrictions may apply. Eligible beneficiaries must use network pharmacies to access their prescription drug benefit except under non-routine circumstances when a network pharmacy cannot reasonably be used.
