Here’s what you can expect with VillageHealth.
Your VillageHealth Nurse (VHN) will assess your needs and develop an individualized care plan for your kidney disease and related conditions. Then your VHN will help you access care (by making appointments or arranging transportation, for example) and act as your coordinator for processes and procedures that require multiple specialties, such as catheter removal, transplant preparation or transitional care following a hospitalization.
Importantly, your VHN makes sure all your doctors, including primary care physician, nephrologist and other specialists, know about your care plan and follow it.
