What to Expect

Patients love the personalized support they receive from VillageHealth, which also helps reduce avoidable hospitalizations and missed treatments. As a physician, you can expect the program to provide an additional resource – an extra set of hands – to help:

  • Coordinate and reinforce the patient's care plan, including care plan changes and discharge orders;
  • Regularly check in with patients; in some areas, the VillageHealth RN or NP will visit patients in their dialysis clinic;
  • Monitor labs and assist patients in tracking key health metrics;
  • Help manage comorbid conditions such as high blood pressure and diabetes;
  • Review medication lists and improve compliance with medication changes;
  • Provide education on kidney disease, treatment options and modalities;
  • Help coordinate vascular access plans;
  • Provide education and support for co-morbidities, diet, nutrition and fluid management;
  • Encourage vaccinations; and
  • Assist patients in managing their diet and nutrition.
