Communication Terms

These communication terms and conditions ("Communication Terms") are applicable only to communications arising out of, and related to, your alignment to a VillageHealth program. By agreeing to receive program communications you acknowledge that DaVita, Inc., its affiliates and subsidiaries, including VillageHealth, DM LLC d/b/a DaVita Integrated Kidney Care (collectively "VillageHealth") and its contracted service providers may contact you via the provided email regarding programs for which you are an attributed member. These terms shall not apply to any other communications. 
 
By checking this box, you acknowledge that you have read and fully understand these Communication Terms and understand the risks associated with email communications from VillageHealth and consent to the conditions outlined herein. Except as provided, VillageHealth is not liable for improper disclosure of confidential information that is not caused by VillageHealth’s misconduct. 
 
You acknowledge that there are risks associated with communicating via email, including, but not limited to the below:
•    While VillageHealth will follow industry standards in sending email communications, the privacy and security of email communication cannot be guaranteed.
•    Email communication may include Protected Health Information ("PHI").
•    After deletion of an email, backā€up copies may still exist on your device.
•    Email is easier to falsify than signed hard copy communications. 
•    VillageHealth cannot guarantee that only the intended recipient can read the email.
 
You further agree to take reasonable precautions when accessing email communications from VillageHealth, including, but not limited to the below:
• Limit or avoid using a third party device.
• Promptly inform VillageHealth of any changes to your email address or communication preferences.
• Preserve the confidentiality of emails by taking reasonable precautions such as safeguarding device passwords.
 
You may opt-out of these email communications anytime by unsubscribing or by reaching out to your care team to update your communication preferences.
