Quality Measures
Information for the following performance measures will be available by February 1st, 2024.
- The Depression Response measure is a patient-reported outcome measure that assesses the percentage of adult patients (18 years of age or older) who score >9 on the Patient Health Questionnaire (PHQ-9), have a diagnosis of major depression or dysthymia and who demonstrate a response to treatment at approximately 12 months.
- The Patient Activation Measure (PAM) licensed through Insignia Health (a Phreesia company), assesses the attitudes, motivators, and behaviors that allow a patient to manage his or her healthcare issues. Patient activation is essential to achieving improved health outcomes and cost control.
- Optimal ESRD Starts quantifies the ability of healthcare providers to prepare patients for ESRD by educating them and their families about the need for dialysis or kidney transplant, helping them to make appropriate and informed choices about their treatment modality, and supporting them in successfully transitioning to dialysis or kidney transplantation as kidney function declines.
Shared Savings and Losses

- Total amount of Shared Savings or Shared Losses per Performance Year
- Proportion of Shared Savings invested in the program and other resources necessary to improve outcomes and reduce Medicare costs for Beneficiaries
- Proportion of Shared Savings to KCE Participants and Preferred Providers for each Performance Year
